IMAGE AND VIDEO CONSENT FORM FOR EVENT ATTENDEES

1. Purpose of Image Processing 

During the event, photographs and videos will be captured for purposes including: 

  • Event documentation
  • Use in event-related publications, websites, and social media
  • Promotion of future events organized by Mirum Pharmaceuticals 

2. Legal Framework 

The processing of your image and personal data will be carried out in strict compliance with: 

  • Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (General Data Protection Regulation – GDPR).
  • Law No. 58/2019 of 8 August (Law implementing the GDPR in Portugal).
  • Portuguese Civil Code.
  • Law No. 41/2004 of 18 August (Law on Privacy in Electronic Communications).

4. Legal Basis for Processing 

Your explicit consent is required for the collection and use of your image and likeness. 

Consent is voluntary, specific, informed, and unambiguous. You may refuse consent without any detriment to your participation in the event. 

5. Rights of the Data Subject 

Under the above legal diplomas, you have the right to: 

  • Access your images and information
  • Request rectification or erasure of your images
  • Withdraw your consent at any time, with effect for future use
  • Object to certain uses of your images

6. Data Recipients and Transfers 

Your images may be shared with: 

  • Event partners and sponsors (if applicable)
  • Third-party service providers (e.g., photographers, media agencies)
  • Public via online platforms and publications 

Images will not be transferred outside the European Economic Area unless adequate safeguards are in place. 

7. Retention Period 

Images will be stored for a period of five years, after which they will be securely deleted unless further consent is obtained.

I freely give my consent for the processing and use of images and videos in which I may appear, as described above. 

Withdrawal of Consent: 
You may withdraw your consent at any time by contacting Martina Risi, Martina.Risi@mirumpharma.com. Withdrawal will not affect any processing already carried out based on your consent before its withdrawal.”
